Community Development Block Grant Residential Survey

Documentation of Residential Impact

The Town of Arcadia is seeking Community Development Block Grant Funding to address the wastewater collection system and treatment plant. The Indiana Office of Community and Rural Affairs requires a residential survey to document the needs from a resident standpoint.  Thank you in advance for providing your input to this survey, it will help the community compete for a $700,000.00 wastewater improvement grant.  Please respond to this survey no later than June 1, 2021. 
1.Please enter the following information for your residential property located in Arcadia, Indiana. 
2.Have you had trouble with your drains and toilet flushing during wet weather events in the past 5 years?
3.If you have issues with flushing your toilet and/or drains, how many times per year does this occur?
4.How long does it take after a wet weather event for your drains and toilets to work properly?
5.Have you ever experienced sewer backups in your home, basement or crawlspace in the past 5 years?
6.If you have experienced a sewer backup, how often has this occurred within the last five years?  Please provide your answer in the comment box below.
7.Have you needed repairs or had to evacuate due to these conditions in the past five years?  The grant requires documentation of residential impact through repairs and costs associated with any damage.  Please provide your answer in the comment box below.
8.Has there been flooding on the street that you live on during wet weather events in the past 5 years?
9.Has flooding occurred on your property in the last 5 years?
10.Have you witnessed any other wastewater issues such as manholes overflowing, sewage in ditches, foul odors from manholes/catch basins, etc.?
11.When you have issues with your toilets, drains, property and/or streets, how much precipitation has fallen?
12.Do you have any pictures, videos or receipts documenting these events in the last 5 years?
13.If you have any health and safety concerns due to the above issues, please describe your concern:
14.In the text box below, please let us know the dates of the events, damage, flooding, etc.  This will help our engineer to compare the date to the Monthly Reports of Operation for the wastewater treatment plant to analyze trends.
