DaVinci Academy of Arts and Science - Parent and Guardian Survey

1.Dear Parents and Guardians,

Thank you for taking the time to participate in this survey. This year, DaVinci Academy of Arts and Science is developing a plan to help shape the future of our school. We cannot stress enough how important it is for us to receive feedback from parents and guardians as we create this plan. Your family and your experience at the school are essential to what we do.

With that in mind, we’d like to share a few important points:

1) This survey is 100% anonymous. Your name is not being collected, and there is no way for us to trace your responses back to you. We have partnered with a third party, Midwest Studies Group, to administer the survey, ensuring complete anonymity. Because of this, we encourage you to be as honest as possible. The more candid your responses, the better we can make improvements for a brighter future.

2) Please complete this survey by noon on February 14, 2025. We ask that only one parent or guardian per household submit a response.
