Acute Ward At Risk patient call back guidelines
The Acute Ward conducts follow up phone calls to "At Risk" patients.
Patients "At Risk' include:
Those not under community care such as HARP or Home Nursing and:
- over 75, lives alone with no supports in place
- dicharged on new complex medications such as Warfarin, multiple drugs, or Clexane
- complex discharge as determined by discharging nurse
- falls risk with a new aid
- self-discharge
- mental health client not linked in with BMH
- post blood or blood products transfusion (excluding those on long term treatments)
