Question Title



My name is Thomas J. Lalka, and I am a student at the University of Phoenix working on a Doctorate in Business Administration. I am conducting a research study The Purpose and Future of Adopting a Quality Model: An Appreciative Inquiry.

The purpose of the research study is to get expert opinion on what is the purpose of adopting a quality model and what do quality experts see as the future of quality modeling. The information will be collected through the use of SurveyMonkey. The study is looking for respondents that are quality experts to answer questions on the purpose of adopting a quality models and the future of quality modeling. A quality expert is defined as a member of ASQ who has been in the quality field for a minimum of five years.

Your participation involves completing the online survey that will take 10-20 minutes to complete. Participants names will not be collected. Once you start, you can withdraw from the study at any time without any repercussions. Your participation will involve completing the survey within seven days. At any time, you can terminate the completion of the survey or not answer specific questions on the survey.

You can decide to be a part of this study or not. The results of the research study may be published, but your identity will remain anonymous. In this research, there are no foreseeable risks to you. Although there may be no direct benefit to you, a possible benefit from your being part of this study is adding to the knowledge in the area of quality at no cost to you.

As a participant in this study, you should understand the following:

1.         You may decide not to be part of this study or you may want to withdraw from the study at any time. Since the participant is anonymous and cannot be identified, a submitted survey cannot be removed from the data collected.

2.         Your identity will be kept  anonymous.

3.         Thomas J. Lalka, the researcher, has fully explained the nature of the research study and has answered all of your questions and concerns.

4.         Data will be kept secure in a password-protected computer file to protect privacy. The researcher's home office is the location for storing all information for three years following completion of the research. At the end of three (3) years, all computer-related disk information will be permanently deleted using a professional electronic disposal service. No hard copy (paper) study materials will be used.

5.         The results of this study may be published.
