Your feedback is very valuable and will help us identify areas that are working well and those that require attention, so we can plan for 2023 and beyond.
What do I have to do?
We would like you to reflect on the past 12 months and provide your honest thoughts on how we performed. We want to hear it all – the positive praise, where we need to improve, and your ideas for the future. We would strongly encourage you to complete this survey together with participants, to make sure their voice is also heard.
How long will it take?
The time taken to fill out this survey will vary, depending on the number of services you access and, therefore, the number of questions that apply to you. Here’s a rough guide:
One service accessed = 7 minutes Two services accessed = 12 minutes Three services accessed = 17 minutes
Can I save and come back to it?
No – this survey has to be completed in one go, so we suggest you find a time when you can complete it carefully and fully in a single sitting.
Is it anonymous?
Yes, the survey is anonymous to protect your privacy.
What if a question doesn’t apply to me?
Most questions do need a response before you can move on. If you don’t feel like a question applies to you, please select the ‘not applicable’ (N/A) answer and you can continue with the rest of the survey.
