Skyline CAP is gathering information for our 2024 Community Needs Assessment to ensure that our community's resources and services align with the needs of all individuals. We value your voice and ask that you please take a few moments to complete this survey. There are 27 questions that should take less than 15 minutes to complete, and your feedback will directly contribute to shaping the future of community support services. Plus – everyone who submits a completed survey will be entered into a drawing for a $50 gift card!
