1. Student Voice Project

This is a short survey to assist the team in determining future planning for positive behavior support strategies in your school.

* 2. Currently what is your grade level status?

* 3. Our school has a small number (3-5)of behavioral rules or examples for the students to follow:

* 4. We were taught what those rules/expectations look and sound like by the staff at school.

* 5. Our school rules are positively stated. We are told what to do instead of what not to do. Example: "Be on time" instead of "Don't be tardy"

* 6. Students who exhibit excellent behavior receive rewards regularly.

* 7. Infractions are clearly defined. We know if we break a rule exactly what will happen every time.

* 8. I know what behaviors will get me sent to the office and what behaviors the teacher will take care of in the classroom.

* 10. What would mean the world to you as a reward from an adult for appropriate behavior? Try to think of rewards that do not involve candy and do not cost money. For example, earning extra lunchtime privileges like sitting at a special table with friends, listening to music in class, a homework free night- things like that.
