Consent Form

Note: This survey will remain open for responses until at least the end of February 2021.

The survey is being conducted by the Brisbane Flight Path Community Alliance (BFPCA), representing Brisbane communities adversely affected by Brisbane Airport’s flight paths.

The purpose of this survey is to collect information on how residents are affected by the network of flight paths over residential areas, how it affects your work, life at home and daily routine.  The survey asks about the effect of aircraft noise on your health and well-being, your engagement in the community consultation process previously conducted by Brisbane Airport Corporation (BAC), and responses to any action you have taken to register your concern.

We will use the consolidated information in preparing a submission to the recently announced investigation by the Aircraft Noise Ombudsman (ANO), and any subsequent submissions to relevant authorities including Airservices Australia, CASA, and The Minister for Transportation and Infrastructure.

You are invited to participate in this survey because you have indicated you’re affected, via your participation in a petition, the Brisbane Flight Path Community Alliance Facebook group, you live in a street known to be under a flight path, or you've directly contacted a committee member of the BFPCA.

Your participation in this survey is voluntary. Of course, you may choose to answer anonymously.  If you share your details with us, we will later inform you of the results of the online survey and subsequent actions.  More than one member of your household may complete the survey, but each person should only complete the survey once.

The online survey will take approximately 10-15 minutes to complete.
