We would like to invite you to provide input into Arbor Health’s 2023 – 2025 Community Health Needs Assessment (CHNA) and subsequent implementation plan. The CHNA process requires an evaluation of the health needs of the community, including a health proﬁle of the community’s population and its health status. It also requires soliciting input from the community regarding unmet needs and priorities.

Your insights and feedback regarding our community’s healthcare needs and gaps will be invaluable to this process.
