Your voice matters! Middlesex Health and community partners would like to hear what you think about health and well-being in your community. Your ideas can help us learn what is important and how we can make the community healthier for everyone.

We would like to learn about the health and well-being needs of people who live or work within Middlesex County (Chester, Clinton, Cromwell, Deep River, Durham, East Haddam, East Hampton, Essex, Haddam, Killingworth, Middlefield, Middletown, Old Saybrook, Portland, and Westbrook) and the towns of Colchester, Lyme, Marlborough and Old Lyme.

When you answer this survey, please think about experiences, the people you know, and others in your community.
