Summary
The Rhode Island Department of Health (RIDOH), UnitedHealthcare, Department of Health and Human Services (Administration for Children and Families), Care Transformation Collaborative of Rhode Island (CTC-RI) and PCMH Kids are pleased to offer pediatric and primary care practices the opportunity to apply for funding to participate in the DULCE learning collaborative.
 
Who can apply: 
Pediatric/primary care practices that provide care to:

a) a steady source of newborns (approximately 150 unique newborns a year)

OR

b) less than 150 newborns a year and a pediatric population that would benefit from services provided by a Community Health Worker (CHW)/Family Specialist (FS) due to high number of health related social needs (HRSN)
OR

c)  partnership with prenatal practice


Practices must also be invested in family-centered, strengths-based communication and care, and seek to collaborate with cross-sector resources to enhance the experience of care for newborns and their families.
 
Applications are due by September 5, 2023 5pm EST. Project activities will begin September 28, 2023 and will continue for 12 months*.
 
*With 3-month extension expected (September 2024 – November 2024)
