Community Needs Assessment

Gratiot Integrated Health Network (GIHN) conducts an annual assessment of mental health needs in our community. This assessment involves public and private providers, school systems and other key community partners and stakeholders.
 
We understand that the mental health needs of our community manifest themselves in various ways.  To better understand these needs, we are asking you, as our community partner, to share with us the trends and needs you identify that may be related to, or indicative of, a mental health need.
 
GIHN is asking that you take a few minutes to answer the following questions.
 
1.What do you see as being the most significant mental health needs that are NOT currently being adequately addressed in our community?(Required.)
2.From your perspective, what trends have you identified that Community Mental Health should be aware of?(Required.)
3.Based on what you have shared, please identify the top THREE concerns/priorities.(Required.)
Thank you for taking the time to provide input. Once we have collected input from our partners and stakeholders, we will develop a prioritized list of those needs we believe we can address. Knowing that community needs require community solutions, we look forward to sharing our results with you and collaborating on solutions.
