This survey is open until 4.00pm Friday 11 December. All answers will remain anonymous.

Background
Since 1995 it has been a requirement under the Disability Services Act (1994) that all public authorities prepare and implement a Disability Access and Inclusion Plan (DAIP) to improve access to their services and facilities for people with lived experience of disability. The Act requires that public authorities lodge a DAIP to the Disability Services Commission.

Since the adoption of the initial DAIP, the City of Armadale has implemented many initiatives and made significant progress towards ensuring that the community is accessible and inclusive for people of all ages and abilities and continues this commitment.

DAIP Review and new DAIP 2021-2026
Currently the City is reviewing its DAIP and invites feedback from people with lived experience of disability, their families and carers to help inform a new 5 year plan.

This survey is part of a process to identify areas of importance when developing strategies around the DAIP outcomes.

We invite you to participate in the survey comprising questions around DAIP Outcomes (listed below) and will take approximately 20 minutes to complete. Please answer only those questions you feel comfortable to answer. None of the questions are compulsory.
