RE: 2020 NCHCFA Annual Awards
It’s time once again for the NCHCFA Annual Awards! You may submit your nomination form online through this survey. This time honored event provides the opportunity to acknowledge front line unlicensed personnel who have demonstrated outstanding achievements in the long term care profession. The Annual Awards program honors individuals who are the backbone in our facilities because of the care they provide to residents. A panel of judges from the Education and Programs Committee will select a Nurse Aide winner from each of NCHCFA’s five districts. In addition, one recipient in a non-nursing discipline will receive the “Riley W. Clapp Outstanding Service” award. Each of the award recipients will be honored at the Annual Awards Gala during the Annual Convention & Expo on Monday, February 24, 2020 in Greensboro, NC.
You will be notified by phone if a nominee from your facility is chosen to receive one of the NCHCFA Annual Awards. Each recipient will be able to invite up to 7 guests to the Awards Gala. The recipient will also be invited to attend all events held on Tuesday, February 25th for “Package Two” attendees at the Annual Convention & Expo. It is important that at least one facility representative attend the dinner with the honoree. If an employee is selected, you will be asked to submit several photographs for use in a special awards presentation. In addition, NCHCFA will visit your facility to produce a video to show of each award winner at the Awards Gala. Please visit our Website at www.NCHCFA.org for video examples of last year's winners. Nominees must not be a previous award winner, and have at least five years of experience in the long term care profession. Please submit only one staff member per discipline.

Please pay special attention to the deadlines. Nominations received in the Association office after Monday, September 30, 2019 will not be considered. Nominations will not be accepted without the appropriate forms. To complete nominations on-line or to obtain additional forms, visit the Member Resources section of the Association Web site at https://www.NCHCFA.org. Click on “NCHCFA Annual Awards Program," and select "Nominations are open for the 2020 Annual Awards Program!"
