Completing this form is required in order to cancel OR substitute teachers already registered but no longer attending Laying the Foundation (LTF) training July 14-17, 2020.
Substitutions:
AdvanceKentucky will formally register the new teacher for LTF.
The substituting teacher will receive email confirmation of his/her registration once the substitution has been processed.
For Cancellations:
Cancellations received prior to July 1st are eligible for 100% credit.
Cancellations received July 2nd - July 7th are eligible for 50% credit.
Cancellations made beyond July 7th will be charged the full registration fee.
No-shows will be charged the full registration fee.
