Community Program Planning Survey for MHSA FY 18/19 Annual Update & Expenditure Plan

This survey is part of the Community Planning Process for the Mental Health Services Act (MHSA) in Amador County.  The purpose of this survey is to hear what you think about the mental health needs and services provided in Amador County. The information you provide will help Amador County Behavioral Health Services design mental health programs that fit the needs of our county.  In order to develop mental health programs that meet the needs of the people in our community, we need to hear from you!

The survey will take approximately 5 to 10 minutes to complete. All of your answers are confidential-we will not be collecting your name or contact information.  You do not have to answer all of the questions in the survey and you may exit the survey at any time.

Below, a quick background on MHSA is summarized for you.

If you have any questions or comments regarding the Community Planning Process or this survey, please contact Stephanie Hess, MHSA Programs Coordinator at (209) 223-6308 or by e-mail at shess@amadorgov.org. We appreciate your input and the time you took to share your experience with us!
