2023 Philly Awards - Order Form for Additional Awards

Congratulations on your Philly Award win! Please complete this form if you would like to order additional awards for any of you collaborators (or for yourself). 

Awards are $78.50 each and invoices will be sent to the individual at the organization / agency placing the order, unless told otherwise. Invoices need to be paid by December 31, 2023. 

Deadline to order: Monday, November 27.

If you would like to purchase more than 3 awards, please resubmit the form as needed, or send your full list to events@npconnect.org.
