Youth Health Services Survey

Thank you for your interest! This survey is being conducted by the Northwestern Health Unit Sexual Health Team. The survey questions will be about your opinion and preferences when accessing sexual health services in the region. Your participation is voluntary. You may choose not to participate. If you decide to participate in this survey, you may withdraw at any time.

 

The survey is 20 Questions long and will take approximately 20 minutes. This survey is completely anonymous and the information you provide is voluntary. If you do complete the survey, the answers you provide will be kept confidential. Since the survey is anonymous, we will not be able to retrieve or remove your responses after they have been submitted. Answers will be pooled together and individual responses will not be shared.

 

This survey will also help us plan future health unit programs and services, within the mandate of public health in Ontario. You can get a copy of the survey results and/or report by calling the health unit or emailing talkpublichealth@nwhu.on.ca.
