Polk County Hazards Public Participation

Thank you for participating in this survey to learn more about natural and human-caused hazards in Polk County. Your feedback is greatly appreciated and will be used to support the newest Polk County Hazard Mitigation Plan. With community participation such as yours, this plan shall help to minimize the impact of major disasters by identifying risks and vulnerabilities before disasters strike. The survey should take about 5 - 10 minutes to complete.

To assist with maintaining confidentiality, respondents are advised not to include any personally identifiable information (PII) or personal health information (PHI) in their free-form responses.
