Self-Direction Conversation for People with Active Plans and Budgets: Pre Survey

This survey is for individuals with active self-direction plans.

For the purpose of this survey, an active self-direction plan is a plan that has been launched and you have started using your self-direction budget. 
Your participation in this survey will assist the Self-Advocacy Association of New York State (SANYS) to be more responsive to the supports and services you need to achieve your goals and dreams for the life you want in your community. This information will also be shared with  the NYS Office for People with Developmental Disabilities (OPWDD) to further conversations regarding self-direction.
