Please provide information to assist us in reaching a settlement with Meridian Health.  If you are using the In-Network level of benefits and you have incurred charges above the $40/$50 co-pay for Primary Care, Specialist, Mental Health Outpatient and/or Chiropractor visits, please document your out of pocket costs here.   You can submit multiple responses through out the year.  

If your costs are likely to be especially high, ie multiple visits that amount to over $300 for single to $800 for family per year, please become active in this fight to hold Meridian Health accountable.  They did not negotiate over these changes, and we are fighting for affordable health insurance for all HPAE members.
