DESCRIPTION:  This survey is intended to assist the Lake Havasu Unified School District to obtain feedback about our schools and gather information about parental involvement.  We value your input and greatly appreciate your cooperation in completing this survey. 
 
INSTRUCTIONS: Please rate the following statements using the rating scale below. If you have more than one child attending our schools, you may return one survey for your family or a separate survey for each child if you prefer to individualize your responses.
