PURPOSE
This form is used to submit issues, feedback, suggestions, support requests, and questions relating to the IPF Nomination and Scoring System.
Submissions must be made by an authorized representative of the submitting authority, including an IPF Executive Committee Member, IPF Member Nation Executive Committee Member, IPF Commission Chair, Regional Federation Executive Committee Member, Regional Commission Chair, or other individual authorized by the IPF or an IPF Member Nation to submit matters on its behalf.
Please complete all applicable sections of this form as accurately and thoroughly as possible. Providing complete information and any relevant supporting documentation will assist in the timely review and resolution of your submission.
