Thank you for participating in this confidential survey. Your responses will assist the Pastor, the deacons, and the Sunday School ministry to determine how to best plan for re-opening the church when we are allowed to do so. Please fill out this survey. If more than one person lives with you, then only one person should complete the survey for the entire household.
