Healthcare Workers Bonus (HWB)— Survey for OPRA & Managed Care Only Providers

This survey applies to providers who may not see Fee for Service (FFS) Medicaid patients but do participate with multiple Medicaid managed care plans to provide services to Medicaid members. This survey also applies to OPRA (Order, Prescribe, Refer, Attend) providers who may be non-payable providers in eMedNY and are seeking a means in which to register for the Healthcare Workers Bonus (HWB).  
1.Are you an Employee or Employer?(Required.)
2.Have you reviewed the Qualified Employer Section of the Department of Health (DOH) HWB Program website as well as the Qualified Employer Attestation to determine if you are a qualified Employer for the HWB Program?(Required.)
3.Do you currently have a Medicaid Management Information Systems (MMIS) Provider ID that is payable through eMedNY?
4.Do you currently have an SFS (Statewide Financial System) Vendor ID?
5.Please provide an email address where DOH can contact you to obtain additional information needed to register for the HWB Portal?(Required.)
