What are we looking for?

- Has developed partnerships that have led to measurable improvements for people receiving care and support.
- Has actively involved the local community, people who receive support, carers, and families in the development and delivery of services.
- Has built strong connections that promote inclusion and strengthen community resilience.
- Demonstrates effective collaboration with partner organisations to achieve shared goals and positive outcomes.
- Has promoted co-production and actively listened to people with lived experience when shaping services or initiatives.
- Has developed partnerships that are sustainable and embedded within ongoing practice.

Can provide evidence of collaborative working with external organisations, community groups, or other partners.
Has established partnerships or initiatives that demonstrate sustainability and long-term benefit.
