Award Nomination Guidelines
To submit a nomination, please ensure your submission meets the following criteria:
Elegibility: Must be an OFSA member or partner
Submission Deadline: Saturday, August 8th, 2026, at midnight.
Supporting Write-Up: You are required to provide a detailed statement explaining why you are nominating this individual or organization.
Please include specific examples and supporting details can be emailed to: info@ofsa.org subject: Award Nomination Backup
Supporting Reference: You must include the name and contact details of one (1) supporting reference.
Selection & Review Process
While the selection committee reviews and considers every submission, awards are granted based on a comparative evaluation of all nominations received. As much as we would love to recognize every single deserving nominee with an award, the competitive nature of the process means that selections must be limited.

| Cornerstone Award The Cornerstone Award recognizes a member funeral home that has made an outstanding contribution to the betterment of the funeral profession at large. |
| Excellence in Funeral Service Award For a funeral professional member or team that has provided outstanding customer service to families. |
| Community Impact Award For a member or organization who has made significant contributions to their local community through their professional work. |
| Rising Star Award For young professionals who are members and have shown exceptional promise and contributions to the industry and who has made significant contributions within their first 1-3 years in the profession. |
| Lifetime Achievement Award Honours a member who has dedicated their career to advancing the industry and who has provided exceptional guidance and support to other professionals. |
| Association Activist Award The Association Activist Award recognizes outstanding contributions to OFSA by association volunteers. |
