This survey is the first in a series designed to update information that is now almost 20 years old on outage causes and cost of distribution systems. It will be used to create one or more white papers that will be available to all participating utilities.

The aggregated data from this survey will be used in one or more IEEE papers and other public presentations to help people understand "why outages happen".

The survey is looking for up to 5 years of data, total counts, not averages please. Please put the range of years for your outage information in the comments box at the end of the survey.
