Thank you for taking the time to complete this 10-minute survey.  BeMe takes a holistic view of things like sleep, nutrition, environment, and other factors to deliver rich insights that help connect the dots between your child and the unknown.

In exchange for completing this survey, we will provide you with a free 12-month subscription to BeMe when we launch in early 2022. The code for claiming your free 12-month subscription will appear after you submit the survey. All of your information is anonymous and confidential.

A platform that allows you to quickly and easily communicate and collaborate with care team members including family, clinicians, therapists, and teachers so that everyone is aligned, and your child is better supported
A Daily Diary that allows you to connect to third party devices (Fitbit, Apple Watch, etc...) and effortlessly log information about your child’s nutrition, sleep, behavior, and other related attributes that may have an impact on their wellbeing
