MPA Hooding Ceremony will be held in the Costantino Recreation Center (CRC) at 10am on June 16, 2017. There is adequate seating for graduates and up to 7 guests but we need numbers to order programs and light refreshments and to provide accommodations for those needing them. Please answer the following questions by May 24th to help us make arrangements.

* Name of graduate attending Hooding Ceremony

* Number of guests (not including yourself)

* Will any of the above guests be in a wheelchair or stroller and need a reserved space? Please provide their names in the box below and we will reserve a space for them.

* Besides the faculty member, will a family member be hooding you who can not reach the stage by stairs?

* A Photography Reminder:
An Evergreen photographer will be taking random photos to give a sense of the event, but not a visual accounting of each graduate. To be sure you have a personal photograph of this special occasion, please have a friend or family member available to take pictures of the graduate.
