The ADPH Awards 2026 will be presented in a ceremony at our Annual Conference on Friday 27^th November. If you would like to nominate a Director of Public Health and/or their public health team for an award, please complete this form.

Please note: You may only submit one award nomination per case or example. If you wish to nominate the same individual or team for more than one award, you will need to complete a separate form for each nomination, using a different case study or project as your supporting evidence.

To make a nomination for an award, please select one of the below options(Required.)

CMOs' National Impact Award - for a DPH in recognition of the impact they have had since starting the role.

Innovative Practice Award - for a DPH who has demonstrated or championed innovative practice within their organisation.

Mentoring Award - for a mentor who has shown dedication and commitment to mentoring as part of the ADPH mentoring scheme.

Partnership Award - for a DPH who has led a successful local or national partnership with a sector or organisation.

Practice Improvement Award - for a DPH-led public health team which has established effective ways of improving public health practice.*

Team Award - for a DPH-led public health team which has worked exceptionally well together to improve public health outcomes in their area.*

Research Award - for a DPH-led public health team for undertaking public health research, or using research evidence in an innovative way, to inform decision making in their organisation.*

*Teams must sit within DPH-led public health functions to be eligible for these awards
