1. Instructions and Introduction to the Survey
Thank you for agreeing to complete the annual State of the Industry (SOI) survey. The survey's findings, in conjunction with other data analysis, will culminate in HCA's annual report on the financial condition of the home care industry in New York State.
Your participation in this survey is essential and will play a key role in the development of our state and federal advocacy plans. Survey findings will be presented in the aggregate, and no individual organization or person will be identified in the report to protect confidentiality.
This year respondents will submit only one survey for their agency which will gather information from each of their entity types (CHHA, LHCSA, Hospice, and MLTC/PACE).
To complete this survey, you will need access to the following report submissions:
- CHHAs and LHCSAs - 2024 Medicaid Home Care Cost Report
- Hospices - 2024 Cost and Utilization Report
- MLTC and PACE Plans - 4th Quarter, 2024 MMCORs
We recommend the following steps to facilitate your survey submission:

- Review all questions to see if you need to delegate certain questions to appropriate staff on your team.
- Once questions are complete, return to this online version of the survey to submit your answers.
- Please note that each question requires an answer before moving to the next page.
