Board Recognition Award Nomination Form

The City of Lethbridge Library Board believes in recognizing and celebrating the contributions that individuals and organizations make towards the Library. These efforts enhance our services, and help us work to ensure that the Library is a welcoming, inclusive space that connects and strengthens our community.

The Board Recognition Award recognizes an individual or organization who has provided a significant contribution to library services in Lethbridge, by helping to advance the mission, vision or values of the Library through their work with the Library and relationship with the community.

• Any individual or organization involved in enhancing one or more services the Lethbridge Public Library offers to the residents of Lethbridge and surrounding area.
• Recipients of other LPL awards are eligible to receive this award.
• Individuals who were employed by the Lethbridge Public Library are eligible to receive the Award.
• Trustees who were members of the Lethbridge Public Library Board are eligible to receive the Award.
• Library staff or Trustees are not eligible to receive the Award until one year after employment with the Lethbridge Public Library or completion of service on the Lethbridge Public Library Board, except where the service is of such value or type that immediate recognition is warranted. In such cases the recommendation will go to the Board with a clear statement of why the one year period should be varied.
• An individual may be named posthumously as a recipient of this award.

The City of Lethbridge Library Board Advocacy Committee will screen all nominations to verify eligibility.

Deadline for Nominations

This award is presented annually when merited. Nominations for the Award shall be accepted up to the first Monday in February of the given year.

The Library Board may confer the award to more than one recipient in any given award year.

Late nominations will not be accepted, but will be carried forward to the following year.

Incomplete or ineligible nominations will be disqualified.

An acknowledgement of receipt of nomination will be made to each nominator via email. If you do not receive acknowledgement of receipt, please contact Kat Short, Executive Assistant, Lethbridge Public Library, at Kat.Short@lethlib.ca.

The Award

A suitable mounted, framed photograph, print or drawing with an embedded plate indicating the recipient’s name, the name of the award, and the year it was awarded. A permanent in-house display recognizing recipients of this award will be housed at either or both of the Main and The Crossings Branches.

Successful nominees, and their nominators, will be contacted by the City of Lethbridge Library Board.

Nominations for recognition are confidential.

Nominations and supporting documents must be submitted by the first Monday in February via this form or a branch of the Lethbridge Public Library to the attention of: Kat Short, Executive Assistant, Lethbridge Public Library.
