BLVD Disciples,

Since March 2020, The BLVD’s COVID-19 Positive Response Team has met weekly to assess developments occurring throughout the course of the pandemic and its impact on our church and community. Now that the vaccine is readily available to persons that are at least 12 years old and both hospitalization as well as positivity rates have drastically declined, the team is prayerfully planning for the appropriate time to resume in-person worship. In preparation, the team would like to obtain some feedback from you to assist them in making a recommendation for re-entry. Please take 2 minutes to complete this survey.
