Vaccine Clinic Volunteers

Thank you for your interest in volunteering at Seguin's vaccine clinics. Please provide your information below. Once completed we will reach out to potential volunteers as we schedule clinics to see if we can fit you in a spot. 
Typical shifts are from 8:45am-1pm and 12:45-5pm.
