INTRODUCTION and THE PURPOSE of THIS RESEARCH

The organizers of the “2017 National Latino HIV/Hepatitis C Conference” and our partners at the Latino Commission on AIDS; Bienestar; National Latin@ Network for Healthy Families and Communities, a project of Casa de Esperanza; ¡Empodérate!; and Coaí, Inc., encourage you to take the "2017 National Survey of Health Care and Social Service Providers Serving the Latino Community." This 15 minutes survey seeks to capture the characteristics, experiences, observations, and perceptions of all those who provide care to Latino clients/patients on an ongoing basis.

The purpose of this research is to collect data from the widest possible range of caregivers serving the Latino Community across the United States and its territories; from the widest possible range of fields, including (but not limited to) HIV, family planning, medical, social service, faith-based, legal, academic, and specialty care; from the widest possible range of care settings; and from service work that is paid or unpaid.

Our goal is to learn from all those who actively promote the health and well-being of the Latino Community.

We apologize that time constraints did not allow the translation of this survey into Spanish; the survey will be translated as soon as possible in order to more comprehensively capture information from all service providers to the Latino population. An initial report on this survey's data will be shared at the “United States Conference on AIDS 2017,” and a more comprehensive follow-up report on the data collected from both the Spanish and the English versions of this survey will then be prepared by October 15, 2017, in observance of National Latino AIDS Awareness Day.

Taking this survey is completely voluntary and all results will be anonymous. Your name will not be used, and your organization will not be identified. You can skip any questions that you don't want to answer.

The survey should take about 15 minutes to complete BUT it is time well spent, as it is the first survey of its kind to measure what service providers are seeing and experiencing when working with their Latino clients/patients.

PLEASE NOTE: If you do NOT provide DIRECT client/patient health care and/or social services, please answer any questions regarding "YOUR" Latino clients/patients based on your KNOWLEDGE of the Latino clients/patients of your organization.
