Listening Session Registration Form 2023
Informed Consent
You are invited to take part in a Listening Session that is designed to talk with Durham County residents about the top health priorities and to collect their solutions and ideas to address them. The Partnership for a Healthy Durham hopes to obtain specific, actionable feedback to use on the 2022-2024 community health improvement plans.
Background Information: The Community Health Assessment addresses a variety of health issues including environmental, social, and systemic factors such as housing, education, prejudice, and pollution. To ensure a healthy future for the entire community we must continue to listen, learn from, and partner with the community to continuously enhance our collective strengths, eliminate barriers to health equity and build multi-sector responses that address our areas of need.
Procedures: If you agree to participate, you will be asked to: *Attend a full 90-minute Listening Session * Complete the online demographic form * Have the audio of the session recorded
Risks and Benefits of Attending a Listening Session: Attending a listening session involves some risk of minor discomforts that can be encountered in daily life, such as stress from the questions. The Listening Session would not pose risk to your safety or wellbeing.
The potential benefit is to determine actionable items to contribute to the current community health improvement plan. Also, the information will help inform the community and Durham County Department of Public Health partners about themes from these sessions.
Payment: A financial incentive in the form of a gift card is associated with completing the Listening Session for those who are eligible.
Privacy: Any information you provide will be kept confidential. We will not use your personal information for any purposes outside of this project. Also, we will not include your name or anything else that could identify you in the reports. The sessions will be audio recorded to capture accurate information.
