2021 Hall of Fame Nomination Form
 
The Memphis Catholic High Hall of Fame was created to honor high achieving individuals associated with Catholic High in all of its iterations, including the original coed Catholic High (1922-1945), Catholic High for Boys (1945-1970), the merged "heritage schools of Sacred Heart High for Girls (1930-1970), Fr. Bertrand High School (1957-1970), St. Augustine High School (1944-1957), and Memphis Catholic High School (1971-2019).

Eligibility requirements for nomination into the Hall of Fame are:
A graduate of Memphis Catholic High School or its Heritage Schools who graduated 10 or more years prior to being eligible for nomination, OR a former faculty member, administrator, or significant supporter of Memphis Catholic High School or its Heritage Schools who worked, or served, at those schools for at least 5 years.

Criteria considered for Induction:
The nominee must have excelled, made an impact, and exhibited leadership in various aspects of life, to include, but not be limited to, education, profession, community, country, and/or personal accomplishments.

Completion Guidelines:
A completed nomination form using the Survey Monkey Option must be finished and submitted no later than April 11, 2021.

If the nominator wants to include additional information to support his/her candidate, e.g., resume, additional letters of recommendations, etc., they can be sent digitally (MS Word is preferred) to: clary@rhodes.edu OR a hard copy can be mailed to the MCHS Hall of Fame Association, P. O. Box 241274, Memphis, TN 38124 and received by no later than April 11, 2021.

A nomination form will be deemed completed when all relevant information has been provided.

If the nominee is deceased, please indicate this next to the nominee's name and leave blank any request for information that doesn't apply. If available, the names and contact information of any close living relatives would be appreciated.
