SGM 2020 Registration - April 17th and 18th - Vancouver, BC - FOR CFMS MEMBERS AND INVITED GUESTS ONLY - Registration due March 12th @ 11:59PM EST

ALL students attending the CFMS Spring General Meeting (SGM) 2020 must complete this form in its entirety.

REGISTRATION FEES: Registration fee is $150.00 per delegate, payable by cash, cheque to CFMS, online via the CFMS website, or in person at SGM 2020. The preferred method of payment is online, you will be directed to the website for payment at the end of this form. This decreases traffic and confusion at the conference itself.

This fee DOES NOT include hotel accommodations. Students must arrange for their own hotels (excluding CFMS-funded delegates, i.e., Travel Award Winners, Board Members, and National Officers).

All SGM 2020 attendees MUST also register for CCME and will be required to pay a $50 student fee. You will be directed to their registration at the end of this form.

CONFIRMATION OF REGISTRATION: Attendees will NOT receive a confirmatory email, as it is not possible using this platform. Please be assured that once you click "Done" your registration is confirmed.

SELF FUNDED student registrations must be accompanied by payment at time of registration or they will not be accepted. Please proceed to our website to pay online.

**PLEASE NOTE: By clicking “Done” you are agreeing to pay the registration fee. Accommodation and meals are charged to the CFMS based on registration numbers collected by the deadline (March 12th) and thus cancellation is not possible. Failure to pay registration fees after confirming registration may result in a letter being sent to the appropriate faculty of medicine
