Carers WA is seeking feedback from WA carers who have experience with the NDIS, to help inform our submission in response to the NDIS Supports rules government consultation.

Survey participants will have the opportunity to win a $50 Prezzee gift voucher.

A carer is a person who provides unpaid care and support to another person who has disability, a medical condition (terminal or chronic illness), mental illness, a substance dependency, or who is frail and aged. This care is not provided as part of a work or volunteer contract, or as part of a course of education or training.

I understand that by completing this survey I am providing consent for my feedback to contribute to Carers WA’s advocacy work.
