Identifying Educator Behaviors that Support Equity in Schools. Participant Consent.

Thanks for completing this form before your interview. This form ensures that you understand and consent to participating in this project, so we cannot proceed until you complete it. 

Please be sure to read the Information Page for this project before you consent to participate. 

If you are under 18 and scheduling an interview for this project, we must receive consent from your parent or guardian before you participate in an interview.  Please ask your parent or guardian to read the project information page and consent to your participation from that page.

1.Please type your name in the space below.                                         **We will not share your name or responses on this consent form with anyone.  We will only record that you consented to participate.  Records of your consent will be kept on a secure drive, and destroyed after one year.

3.Project description. I have read and understand the project webpage. (Required.)
4.Interview Process.  Before the interview begins, the interviewer will  describe the project, and ask if you have questions. We will not proceed, or begin recording, until you say you are ready.

The interview will begin with this question,

"How have educators (e.g. teachers, counselors, coaches, librarians, nurses, administrators, etc) behaved that have made it more possible for you (or your child) to thrive and learn in school, and behave in ways that allow others to thrive and learn?

Once you answer that question,  the interviewer will ask follow up questions to learn more.

You may ask for a pause or stop the interview at any time. You may choose not to participate at any time.

5.Quotes and confidentiality:

Our report of the findings of this research will be available publicly after March 1, 2024.  We'll report on patterns, trends, and common practices, not individual responses.  But to describe those patterns, trends and practices, we will include illustrative quotes, which we will alter as needed to ensure the quoted participant is not identified.

You have options about how or if we quote you in the report.
