The survey should be completed by your organization’s Executive Director, CEO, or another individual authorized to make decisions on behalf of the organization. It should take approximately 10-15 minutes to complete.

- 24/7 access to care youth workers can use anytime
- Confidential, convenient support from licensed providers including virtual urgent care and mental health services
