The Healthy Communities Team at Bellarine Community Health are collecting information about food access to help inform future health promotion work and planning. This survey will be asking you:
- Where you get your food from
- What you eat
- What makes it hard and what makes is easy for you to get food
We are only asking for people who live permanently in the 3223 postcode (Portarlington, St Leonards and Indented Head) to complete the survey. If you do not live in these areas you do not need to fill out the survey.
Thank you for taking the time to complete this survey, it will take approximately 10 minutes and there are 20 questions. You can exit the survey at any time.
