As Covid-19 cases and positivity rates continue to rise in the city and state, in large part due to the new Delta variant, we want to continue to hear from you as we make decisions concerning in-person worship gatherings. Please complete this brief survey as we continue to be prayerful about worship in the Fall.

I am primarily attending weekly worship services in the building

I am onboard with requiring proof of vaccination for in-person worship offerings but would not continue to attend until the sanctuary was open for all
