QUESTIONS FOR Ministry Leaders
To Better Serve Leaders Like You!

I am seeking your participation as a ministry leader in a brief survey.  I know that a large portion of your time is spent counseling, whether you are in a full-time, part-time, or lay leader position.  I would like to get feedback from your experiences in this area.  Your responses to this survey will help me evaluate what might be effective tools that would be an asset to you as your work with individuals and families.

The survey is very brief and should only take about five minutes to complete.  Your responses to this survey will be kept completely confidential.

Thank you very much for your time and cooperation.  Your feedback will be very helpful.

Sincerely,

Corey J. Gilbert, PhD, LPC
