Making healthcare premiums and out-of -pocket costs more affordable

If your premium could be lowered, would you be willing to sign up for a plan that included:

* 2. Coverage only for treatments whose effectiveness and value is proven?  For example, this type of plan might not cover a $300,000 treatment that has only been shown to improve life expectancy for a very short time, such as one month.

* 3. A plan with lower premiums for members who have a living will with instructions for no heroic end-of-life measures?

* 4. A plan with wellness requirements?  For example, this type of plan might require an annual physical, recommended preventive services (i.e. colonoscopy for members over 50) and/or monitoring and coaching for certain chronic conditions.

* 5. A plan with a narrow network of providers?  For example, this type of plan may require that you use a certain hospital for all inpatient care.
