 A survivorship care plan is a summary of your cancer diagnosis (e.g., type and stages) and treatment history (e.g., type, side effects, and dates). It includes information on managing ongoing symptoms and possible long-term effects of your treatments, future checkups, follow up tests and surveillance for cancer recurrence, tips on cancer prevention, suggestions for maintaining a healthy lifestyle (e.g., nutrition and exercise), and possible psychosocial impact of cancer diagnosis and treatment.

Are you a breast cancer survivor of African descent who: completed breast cancer treatment a minimum of 6 months prior to completing the survey, breast cancer is in remission/cancer free, received a survivorship care plan at the end of treatment, 18 years of age or older, able to provide informed consent, able to read, understand, and write English and resides in the United States?
