Plain Language Statement

Purpose of this survey: The Federal Member for Solomon has commissioned this survey to gain an in depth understanding of the types of services and resources veterans and emergency service workers wish to have in Darwin and Palmerston to support themselves and their families. 
 
Benefits of this survey: This is your opportunity to provide anonymous feedback on the services and supports you believe are lacking or needing development. All feedback will be taken on board with a focus on delivering. 
 
What do we expect from you: Voluntary participation will include answering some basic questions such as your age bracket and gender. You will also be asked what types of services and supports you like that are currently available, and other services and supports you would like to see available. The survey will take less than 5 minutes. 
 
Risks: There are no known identified risks associated with participating in this survey, however if you do experience any distress please discontinue the survey and contact Lifeline on 13 11 14. 
 
Confidentiality: The data you provide will be confidential and stored on a secure server. At the end of the survey, you will be asked if you are interested in participating in a future one-off survey investigating mitigation methods for PTSD. Should you wish to participate in this study, enter your email address for future contact via email. All email addresses will remain confidential and be securely stored.
