PARENT/GUARDIAN TRANSPORTATION WAIVER

* 1. As a parent/guardian of a youth in the SKORE program, I hereby grant permission to the SKORE Director and any approved SKORE Mentor(s) to transport my child in their personal vehicle.  As such, I agree to waive any future claim against the SKORE program, and anyone associated with the program, in the unlikely event of an accident occurring during the transport of my child(ren).
