The Woolwich Community Health Centre has identified seniors, families, youth, and farm families, including those from the Mennonite communities, as priority populations. This health and wellness survey assessment will focus on the needs of these populations and the Woolwich and Wellesley communities in order to enhance the existing educational and support groups/ programs.
Health and wellness programs and/or groups include activities that enhance awareness and personal skills, and change behaviour (through education, information and group support) to improve the health of seniors, families, the farming community and other groups and communities. This assessment also helps Woolwich Community Health Centre to better know the Woolwich and Wellesley communities and respond to emerging needs to promote health and wellness.
In order to plan for future health and mental wellness education and support groups, we are asking you to help us by answering the following questions:
